Health Related Social Needs Services
Health Related Social Needs (HRSN) services allow qualifying Apple Health (Medicaid) enrollees to receive evidence-based, non-medical services aimed at supporting individual health outcomes. Seattle/King County Aging and Disability Services will provide HRSN services to Apple Health enrollees who are not receiving Long-Term Services and Supports (LTSS) through June 30, 2028. Nutrition Support services are currently available and Home Modifications, Remediations, and Adaptation Devices will be available in July 2026.

Who Can Get These Services?Who Can Get These Services?
While each HRSN service has specific eligibility criteria, an individual must meet the following general three basic requirements:
- Be enrolled in Apple Health (Medicaid).
- Meet the social risk factor criteria such as:
- Experiencing food insecurity or,
- Living in a home that is physically unsafe, inaccessible, or negatively impacting their health.
- Meet the clinical risk factor criteria by:
- Living with a chronic condition(s) like a medical or behavioral health condition that persists over time.
These services are free to all qualifying individuals.

Nutrition Supports
Nutrition supports are designed to address low or very low food insecurity as outlined by the USDA guidelines. This program offers several options based on your health needs:
- Nutrition Education and Counseling: Strategies and guidance from nutrition experts to help motivate adoption of food choices and learn how to use food to feel your best.
- Medically Tailored Meals: Fully prepared, healthy meals delivered directly to your home and tailored to your health conditions.
- Fruit and Vegetable Provisions: Fresh, frozen, or canned fruits and vegetables from participating food retailers and farms.
- Pantry Stocking: An assortment of healthy grocery staples, fruits, vegetables, grains, and proteins to help improve your nutrition.
Home Accessibility Modifications, Remediations, and Adaptation Devices
These support services provide physical changes to your home that are necessary to address your health, welfare, and safety or enable you to function with greater independence in the home.
- Home Accessibility Modifications: Address known home-based health and safety risks and support the occupants’ health and safety in the living environment. Examples include ramps and grab-bars, wheelchair access improvements, and non-skid surfaces.
- Home Remediations: Address hazards and ensure a safe living environment. Examples include ventilation improvements, minor mold removal, and de-humidifiers.
- Home Adaptation Devices: Provision, delivery, and installation of clinically appropriate devices to support the treatment or prevention of medical conditions. Examples include air conditions, portable power supplies, and refrigeration units.
