2017 Care Transitions: We’re All In It Together
Health and human services providers from throughout the Puget Sound region gathered to discuss patient- and family-centered transitions of care across the continuum at “Care Transitions: We’re All In It Together” on May 10, 2017 at the Four Points by Sheraton Seattle Airport South (22406 Pacific Hwy S, Des Moines, WA). The focus of the 2017 conference was developing a shared vision for patient- and family-centered transitions of care across the continuum. The conference keynote speaker was Tim McNeill, RN, MPH. 2016 Care Transitions: Charting a Course to Health
The 2016 Care Transitions Conference—Charting a Course to Health—took place on Tuesday, May 17, at IAM751 (9125 15th Place South, in south Seattle). The conference drew nearly 250 participants from more than 75 organizations. Registration was open to anyone engaged in improving health care quality and transitions of care, including hospital and home health care administrators, discharge planners, nurses, physicians, pharmacists, policymakers, social service providers, consumers, community leaders, and others whose role involves empowering patients and families to manage their health care.
