If you work with older adultsā€”as a service provider, caregiver, health care professional, or advocateā€”mark your calendar for this yearā€™s care transitions conferenceā€”Care Transitions: Weā€™re All In It Togetherā€”on Wednesday, May 10, 2017, from 8 a.m. to 4 p.m. at the Four Points by Sheraton Seattle Airport South in Des Moines, Washington.

This is the sixth year that hospital and home health care administrators, discharge planners, nurses, physicians, pharmacists, policymakers, social service providers, community leaders, and consumers will gather to discuss new ways to work together to decrease avoidable hospital admissions and health care utilization.

Hospitalizations contribute significantly to the cost of health careā€”costs borne by insurance companies, taxpayers (Medicaid and Medicare), patients, and familiesā€”and take an emotional toll on patients and their families.

The focus of this yearā€™s conference will be on developing a shared vision for patient- and family-centered transitions of care across the continuum. We encourage participation from stakeholders throughout King, Snohomish, Pierce, and Kitsap counties, while welcoming anyone engaged in improving health care quality and transitions of care. Based upon previous participation, we anticipate that 300 individuals will attend the 2017 conference, representing more than 75 organizations.

The annual care transitions conference is coordinated by Aging and Disability Services with support from a community-based planning team.
